1 Basic Verification

Verify your personal information is correct

Name, date of birth, insurance ID, address

Check the date of service

Does it match when you actually received care?

Verify the provider name and facility

Is this the correct doctor/hospital that treated you?

Confirm your insurance was billed correctly

Check that in-network rates were applied if the provider is in-network

2 Charges Review

Request an itemized bill

If you only have a summary, call and request a detailed line-by-line breakdown

Check for duplicate charges

Look for the same service listed twice or overlapping charges

Verify you received all listed services

Were you actually given every medication, test, and procedure listed?

Check for upcoding

Was a more expensive procedure code used than what was actually performed?

Look for unbundling

Services that should be billed as a package being charged separately at higher rates

Verify operating room time

If applicable, check that OR time matches your actual surgery duration

3 Insurance & Coverage

Compare with your EOB

Does the bill match what your insurance said you owe on the EOB?

Verify deductible tracking

Is the correct deductible amount being applied? Has it already been met?

Check for denied claims

Were any services denied? If so, consider filing an appeal

Verify coordination of benefits

If you have two insurance plans, was the secondary plan also billed?
